Introduction

The single most difficult problem in esthetics is acne. That is because there is no single, universal, sure-fire treatment.  For further discussion, please visit our Knowledge Base.

Acne tends to be a management problem, it is unusual to see a patient arrive, clear up quickly and never be seen again. The problem comes in cycles and stress is often at the root of the problem.

Adolescent acne differs from occasional acne or breakouts in mature adults.

Persistent acne into middle age requires a careful history workup to determine what has been done in the past and what is used now.

Many acne problems are a direct result of bad skincare products or ingredients. For example, acne rashes, or acneiform rashes, especially as found in rosacea can almost always be traced to improper treatment choices.

One of these is the over use of benzoyl peroxide, a very potent free radical generator, which tends to aggravate inflammation. Its use should be restricted to the studio and extremely limited, temporary home care.

Acne and inflammation often occur together even if treatment choices are not at issue.

Your best route is to address the inflammation issue first. When you reduce that, the acne will tend to reduce also. Too often in medical acne therapeutics the topicals are not well tolerated or over applied and actually aggravate the condition. This phenomenon is observed much more often than reported in the literature.  
 

The standard medical classification for acne is useful to guide your treatment selections using 302:

ACNE CATEGORY 1: Mild Acne - Comedonal


Comedones with some reddening, some rash like areas.

Fewer than 20 comedones OR fewer than 15 inflammatory lesions OR total combined less than 30.


ACNE CATEGORY 2: Moderate Acne - Papulopustular

Lots of comedones and inflamed, rashy areas.

20-100 comedones OR 15-50 inflammatory lesions OR total lesion count from 30-125.


ACNE CATEGORY 3: Severe Acne - Nodular (Cystic)

Cystic type acne marked by lesions and nodules and comedones, pus filled.

More than 5 nodules OR comedones plus inflammatory lesions greater than 50 OR total lesion count greater than 125.

Mild Acne: Comedonal (Acne Category 1)

Comedones with some reddening, some rash like areas.  Fewer than 20 comedones OR fewer than 15 inflammatory lesions OR total combined less than 30.


HOME CARE REGIME

Mornings
Apply Sensitive Cleanser to dampened skin, massage gently, rinse very well. Pat dry. This should be a quick wash.

Apply Acne Mist immediately after cleansing, follow with Recovery Minerals (SPF-30) for makeup. Other mineral makeup may cause irritation.

Lotion sunscreen is not advised except for sustained sun exposure, then use SPF-15 or SPF-30 and reapply as needed. Other sunscreens may cause irritation.

Do not use liquid makeup of any type.
    

Evenings
Remove mineral makeup (if any) with tissues and apply a hot, damp towel to face, allow to cool for 1 minute.  Apply Sensitive Cleanser to dry skin and massage in well, as if a lotion. Add lather from Face & Body Bar and warm water, massage well and rinse very well.

Variation: If breakouts are occasionally widespread, dip dampened fingers into baking soda and massage into area gently while still lathered. Do not scrub, allow to dissolve.  Do not apply more often than 3 x per week. Rinse well.     
            
Apply Acne Mist immediately after cleansing, follow with our Recovery Minerals (SPF-30).

Variation: After three weeks and if condition has not markedly improved, choose either 302 Serum or 302 Acne Drops.  Choose 302 Acne Drops if skin tends to be dry and rashy, otherwise choose Serum and apply before Recovery Minerals (SPF-30).

If using 302 Acne Drops, apply before adding cleansers and massage in. The purpose is to solubilize sebum AND condition and renormalize skin. Rinse very well after applying cleansers.  Do NOT apply as a leave-on afterward. Apply every evening or every other evening.


Regime Goal:  Skin should feel soft, supple and not dry.  Reduction in visible acne will follow.    
        
AVOID: ALL LOTIONS, LOTION MAKEUP and CREAMS
DO NOT MIX 302 WITH NON-302 PRODUCTS EXCEPT AS INDICATED

Moderate Acne - Papulopustular (Category 2)


Lots of comedones and inflamed lesions and some rashy areas.  20-100 comedones OR 15-50 inflammatory lesions OR total lesion count from 30-125

HOME CARE REGIME

Mornings

Apply Acne Cleanser to moistened skin (shake bottle first), massage gently, allow to set for 30 seconds; rinse very well, under the shower best. Pat dry.
    
Apply Acne Mist immediately after cleansing, follow with Recovery Minerals (SPF-30) for makeup. Other mineral makeup may cause irritation.

Lotion sunscreen is not advised except for sustained sun exposure, then use SPF-15 or SPF-30 and reapply as needed.  Other sunscreens may cause irritation.  Do not use liquid makeup of any type.
    

Evenings        


Remove mineral makeup (if any) with tissues and apply a hot, damp towel to face, allow to cool for 1 minute.  Apply 302 Acne Drops and massage in well. Apply Face & Body Bar lather directly  over  this massage around face and 2x per week add to that baking soda. Dip fingers in baking soda and smooth in, do not scrub, rinse very well. Pat dry.
    
Apply Acne Mist immediately after cleansing, follow with our Recovery Minerals (SPF-30) to aid in healing and reduce inflammation.

If skin tends to feel too dry, eliminate baking soda application. If dryness continues, reduce frequency of application of Acne Cleanser and use Face & Body Bar only.    

Acne Mist may be used as often as needed for hydration.


Regime Goal:  Skin should feel soft, supple and not dry. Reduction in visible acne will follow.

Cystic type acne marked by lesions and nodules and comedones, pus-filled.  More than 5 nodules OR comedones plus inflammatory lesions greater than 50 OR total lesion count greater than 125.

HOME CARE REGIME

Mornings

Apply a light amount of Face & Body Bar lather, rinse well. Pat dry.

Apply Acne Mist immediately after cleansing, follow with our Recovery Minerals (SPF-30) for makeup. Other mineral makeup may cause irritation.

Lotion sunscreen is not advised except for sustained sun exposure, then use 302 SPF-15 or SPF-30 and reapply as needed. Other sunscreens may cause irritation.  Do not use liquid makeup of any type.
    
Evenings    
    


Apply a more generous lather to the face and a small amount of baking soda to the fingertips, smooth in (do not scrub) and rinse well. Pat dry. Do not exceed 3 times per week application of baking soda.

Apply Acne Mist immediately after cleansing.

Apply 302 Serum and smooth in.

When serum has fully absorbed, apply Recovery Minerals (SPF-30) to aid in healing and reduce inflammation.

Acne Mist may be used as often as needed for hydration.

Regime Goal:  Skin should feel soft, supple and not dry. Reduction in visible acne will follow.

How to use 302 products with topical and oral antibiotics, medicated cleansers, retinoids, Accutane

HOME CARE CONSIDERATIONS

The primary benefit of 302 used as an adjunct to prescriptive medications for acne is 302s mildness and synergy. Many times prescriptive medications are difficult to properly dose per patient. With 302 the prescriptive medication potency may be decreased as well as their application frequency, which gives a better overall outcome and a better rate of toleration.

Medicated cleansers for example often are not well tolerated. Irritation can be a significant problem when these are used in conjunction with or contain benzoyl peroxide.  Discontinue medicated cleansers if irritation is present and introduce 302 Cleansers. It may take a week or longer for the patient to see a reduction in ongoing discomfort after introducing 302.

When leave-on prescriptive products, especially the topical retinoids or any combination of topical retinoid and an antibiotic, such as benzoyl peroxide, doxycycline, clindamycin, etc. are in play, and inflammation caused by poor toleration of the topical product has not subsided within one month or has become worse, it will be necessary to reduce frequency of application and introduce 302. It may be necessary to temporarily discontinue use altogether of these prescriptive medications for patients who have demonstrated poor tolerance to them.

When the skin has stabilized with 302, generally after two weeks, the prescriptive products may be re-introduced but at less frequent application (for example two or three days per week versus everyday).

When considering 302 as part of a broad program, best results occur when treating the inflammation first.  Provide mild 302 cleansers. Alternating between a prescriptive cleanser and 302 Sensitive Cleanser or Face & Body Bar often works quite well. Advise the patient that the goal is to avoid the dry, squeaky, clean feel after washing. Skin should be soft and supple, not parched. Dryness leads back to inflammation.

It is inadvisable to provide acid peels of any type to skin that currently suffers from intolerance to a topical regime, prescribed or otherwise.  Reduce the inflammation and you often reduce the incidence of lesions and comedones. Cleansing is key to this. The use of salicylic acid to reduce lesions is not recommended when irritation caused by topical products is the cause.

After the inflammation has subsided, proceed with exfoliation as required. It is acceptable to combine 1 part Exfoliator Plus to 4 parts Exfoliator in the 302 Acne Facial, especially if hyper-keratinization or seborrheic dysplasia is evident (often called milia or small kernels of waxy hard sebum) OR very oily secretions, often pus filled. This facial treatment is recommended weekly.

Benzoyl peroxide applications should be restricted by the physician to two or three times per week when using 302 and this is a good rule of thumb even without 302. As a general rule, whenever any prescriptive products, oral or topical are in play, use the 302 regime found in Acne Category 3 and intermix, as needed with the prescriptive product.

When using Accutane, the skin often is fragile and dry. 302 is very compatible with Accutane.  The following regime avoids the pore clogging, high barrier petrolatum and mineral oil creams that while temporarily effective in curbing Trans-Epidermal Water Loss (TEWL) may also irritate the skin from constant contact.

The emphasis with 302 to address Accutane side effects is to provide an ultra mild cleansing and the healing activity of zinc oxide coupled with high activity natural moisturizers that are universally well tolerated. Advise patient to avoid lotion makeup, it can often be very drying and pore clogging and irritating.

HOME CARE REGIME

Mornings

Wash lightly, gently. Apply a light amount of Face & Body Bar lather, rinse well. Pat dry.


Apply Acne Mist immediately after cleansing.

Apply SPF-15.

Optional: Apply Recovery Lotion Rx over SPF-15 if skin is very dry.
    
During the Day

For hydration as needed: apply Acne Mist or Recovery Lotion Rx and Lip Balm as desired.

Evenings       

Apply Recovery Cleanser Rx to skin (no water).  Massage in gently. Apply a second amount and water and massage gently, rinse very well. Pat dry.

Apply Acne Mist immediately after cleansing.

Apply Recovery Lotion Rx.

AVOID: ALL LOTIONS, LOTION MAKEUP AND CREAMS
